    4. Death Benefit - $10,000. This benefit is for the member only.
    5. Accidental Death, Dismemberment and Loss of Sight Benefit - $10,000
    6. Podiatry Benefit
      The Podiatry Benefit is provided at the DC 37 Health Center at 115 Chambers Street in Lower Manhattan (212) 766-4455 and 186 Joralemon Street in downtown Brooklyn

    7. Audiology and Hearing Aid Benefit
      The Audiology benefit is provided at the DC 37 Health and Security Plan Audiology Center only, which is located at 125 Barclay Street (Room 770), New York, NY, (212) 341-4790. The benefit is available to members only.
    8. Supplement to the GHI/CBP(New Catastrophic Medical benefits)
      This benefit is available only to eligible active employees and their dependents who are enrolled in the GHI/CBP Blue Cross program. Eligible members will have a $1000 out-of-pocket expense before being reimbursed.
